2. No Doctor-Patient Relationship; Informational Purposes Only

The Site, including all content describing our services (General Family Dentistry, Emergency Dental Services, Cosmetic Services, and Aesthetic Treatments such as Botox, Xeomin, and Juvéderm), is provided for general informational purposes only. It is not intended to be, and should not be treated as, dental or medical advice, diagnosis, or treatment.

  • Use of the Site, including submitting a contact or appointment request form, does not create a doctor-patient (or dentist-patient) relationship between you and Grams Family Dental or any of its providers.
  • A doctor-patient relationship is only established once you have been seen in person (or via a scheduled telehealth encounter, if offered) and formally accepted as a patient of the practice.
  • Nothing on the Site is a substitute for professional dental or medical evaluation, diagnosis, or treatment. Always seek the advice of a qualified dentist or physician with any questions you may have regarding a dental or medical condition.

Do not use online forms for medical or dental emergencies or to disclose sensitive health information. If you are experiencing a dental or medical emergency, call 911 or go to your nearest emergency room. For urgent dental concerns, please call our office directly at (414) 321-6890.

3. Appointment Requests and Scheduling

The Site may allow you to request an appointment through our online form or through our third-party scheduling platform (currently provided by NexHealth). Please note:

  • Submitting an appointment request through the Site does not guarantee you an appointment; all requests are subject to confirmation by our staff.
  • We ask that you not submit sensitive medical history, insurance details, or other protected health information through the general contact form. Please discuss such information with our office by phone or in person.
  • Appointment scheduling, reminders, and related communications may be facilitated by third-party vendors (such as NexHealth) who have their own terms of service and privacy practices, which we encourage you to review.
  • Cancellation policies, missed-appointment fees, and related office policies will be communicated to you separately and are not governed by these Terms.

4. Communications Consent (Email & SMS)

By submitting a form on the Site or otherwise providing your contact information, you consent to receive email and/or SMS text messages from Grams Family Dental related to appointments, office communications, and general updates. Message and data rates may apply. You may opt out of SMS communications at any time by replying “STOP,” and you may unsubscribe from emails using the link provided in any message, or by contacting us directly at office@gramsfamilydental.com.

5. Use of the Site

You agree to use the Site only for lawful purposes and in accordance with these Terms. When using the Site, you agree not to:

  • Provide false, misleading, or fraudulent information in any form on the Site;

You agree to indemnify, defend, and hold harmless Grams Family Dental and its owners, providers, employees, and agents from and against any claims, liabilities, damages, losses, and expenses (including reasonable attorneys’ fees) arising out of or related to your violation of these Terms or your misuse of the Site.

12. Privacy and Protected Health Information

Your use of the Site is also governed by our Privacy Policy. Please note that general web forms on the Site are not a secure or HIPAA-compliant method for transmitting protected health information (PHI). Communications regarding your medical/dental history, treatment, billing, or insurance should be made by phone or through a secure patient portal, if available, rather than through the general Site contact form.

The Site is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Site. Appointment requests for minor patients should be submitted by a parent or legal guardian.

These Terms are governed by and construed in accordance with the laws of the State of Wisconsin, without regard to its conflict of law principles. Any disputes arising out of or relating to these Terms or your use of the Site shall be subject to the exclusive jurisdiction of the state and federal courts located in Milwaukee County, Wisconsin.
